Coaching Eligibility Forms

• New Coaches need to complete a GBGSL Coach Eligibility Disclosure form and a Vermont Release Form and return them by April 1st, 2010 to GBGSL Administrator, PMB 127, 150 Dorset St., Ste 245, South Burlington, VT 05403 to be processed. Please note that the forms need to be notarized - a notary will be available at the first coaches' meeting.
• Veteran GBGSL Coaches - New This Season: If you have coached in GBGSL in the past and we have your information on file, we will re-submit your Vermont Release Form to the Vermont Criminal Information Center to update our records: you are not required to complete this form each season. However, please be sure we have your current phone number and email address on your daughters registration form.

Expectations of Coaches

• general knowledge of soccer
• organize practices and games
• initiate contact with parents
• help set up and take down goals
• maintain equipment bags
